How to Apply for SSS UMID (UNIFIED MULTI-PURPOSE ID)? Steps and Requirements for UMID

The Unified Multi-Purpose ID or UMID is one of the most applied for identification cards by Filipinos.
It’s a powerful ID that is a product of the collaboration of 4 government agencies including the SSS (Social Security System), GSIS (Government Services Insurance System), Pag-IBIG Fund, and PhilHealth.
The UMID is used to transact with these agencies including the availment of benefits and application of loans. And with the new UMID card, members can now use it to withdraw their benefits and loan proceeds through the affiliated ATM machines.
The UMID card is considered a primary valid ID and it is generally accepted when doing transactions with both the government and private offices. It is also one of the primary documents that job agencies require their applicants to submit when applying for jobs abroad.
That all said, it may be a good time to get the new UMID and in this article, we’ll share with you the requirements and steps on how to get UMID card.
REQUIREMENTS
For First Time Application
The member must have no existing UMID Card issued by the GSIS or PhilHealth.
The member must have at least 1 monthly contribution and a valid date of coverage.
The member’s information must be the same in the UMID Card Application and SSS Membership database. This includes the SS Number, Name (First Name, Middle Name, Last Name), Date of Birth, and Gender.
The member must have a valid ID as proof of identity. You must present the original ID and submit the photocopy of the ID.
Accepted Valid IDs and Documents
You may present any 1 of the following primary IDs to SSS when applying for UMID Card.
Driver’s License
Valid Passport
PRC Card
Seaman’s Book
If you don’t have the above IDs, you may submit any 2 of the following IDs or documents but make sure they bear your correct information including your name and date of birth:
Postal ID
Marriage Contract/Marriage Certificate
NBI Clearance
PhilHealth Member’s Data Record
Police Clearance
Pag-IBIG Member’s Data Form
School or Company ID
TIN Card
Membership Card issued by Private Companies
OWWA Card
Senior Citizen card
Voter’s ID card/Affidavit/Certificate of Registration with COMELEC
Alien Certificate of Registration
ATM card with cardholder’s name; or bank certification if without cardholder’s name
Birth Certificate
Bank Account Passbook
Baptismal Certificate
Credit card
Firearm License Card issued by the PNP
Fishworker’s License issued by BFAR
GSIS Card/Member’s Record/Certificate of Membership
ID Card issued by LGUs (e.g., Barangay/Municipality/City)
ID Card issued by professional associations recognized by the PRC
Life Insurance Policy
Seafarer’s Registration Certificate issued by POEA
Certificate of Licensure/Qualification documents from MARINA
Student Permit issued by the LTO
Transcript of School Records
Certificate from National Commission on Indigenous Peoples or Office of Muslim Affairs
Certificate of Naturalization from BI
Birth/Baptismal Certificate of child/children or its equivalent
Health or Medical Card
For UMID Card Replacement
If you want to replace your UMID card because it has wrong information, you need to surrender the old SSS ID or UMID card.
If you lost your UMID card or you did not receive it at all, you may submit a duly-notarized affidavit of loss or affidavit of non-receipt of the card.
The UMID Card Replacement Fee is Php 200 which applies to the following:
Correction or Change of name (example: from maiden name to married name)
Correction of date or place of birth and/or sex
Replacement of lost or damaged card
Change of home address
Change of authenticating finger due to amputation or sickness (example: index to middle finger)
Update of member’s photo
Change of old card to new card
Change of specimen signature printed on the card
The reduced replacement fee took effect on December 11, 2017, based on SSS Office Order 2017-066. It was Php 300 before.
EASY STEPS WHEN APPLYING FOR A UMID CARD
Step 1 – Prepare all the requirements for your UMID Card application. Please refer to the list of valid IDs and documents above.
Step 2 – Go to the nearest SSS Branch that has UMID enrollment or ID Capture facility. See the photo below for your reference.
Step 3 – Once you’re at the SSS branch, get a copy of the UMID Card Application Form from the Information Personnel and fill it out with the complete and correct information.

Take note of the following when filling out the form:
Fill out all the required fields in the UMID Card Application Form. Double check the information you entered to make sure they’re all correct. Avoid erasures and alterations.
Indicate “N/A” or “Not Applicable,” if the required data is not applicable.
Provide at least 1 contact information detail such as mobile phone number, home landline or email address where SSS can notify you of the status of your card application.
If you wish to enroll your UMID as an ATM card, you must specify the preferred SSS-accredited bank on the application form. (For now, only UBP or UnionBank of the Philippines is available).
For your address, indicate your permanent address instead of the temporary mailing address.
Step 4 – The SSS Personnel will verify your UMID card application and your requirements, then you will be instructed to proceed to the ID and Biometric capture booth. You will also have to affix your electronic signature in that same booth.
Once these processes are done, you will be given an acknowledgment slip with the estimated delivery date of your UMID card.
Step 5 – Wait for your UMID Card to be delivered to your mailing address.
The verification and printing of the UMID card usually take 8 days, then it will be delivered via Philpost within 2 weeks for NCR and 4 weeks for those with registered addresses outside NCR.
The normal turn around time for the member to receive the UMID card is within 30 days upon successful application.
Step 6 – Once you have received your UMID card, you can then go to the nearest SSS branch or kiosks to activate your UMID Card. For UMIwD-ATM Cards, you need to go to the accredited bank to activate it.
Use your UMID Card as ATM
Aside from the UMID Card being one of the primary and valid IDs in the country, you can now also use it as an ATM (Automated Teller Machine) card.
As an ATM card, you just need to enter it in the automated teller machines to withdraw your social security benefits such as disability, retirement, and loans.
For self-employed and voluntary members, your sickness and maternity claims can be directly disbursed to your UMID card.
For employed members, your sickness and maternity benefits will still be coursed through your employer.
Features of UMID Card ATM Account
Everything has been made easier with the new features of the new UMID card.
1. It’s very convenient because you need not go to the SSS office to pick up your check or wait for your check to be delivered to your address, then you still have to go to the bank to encash it.
2. It’s cost-efficient and safer because it will eliminate the cost of printing and mailing of checks, as well as the delay in the delivery of service due to postage time and lost checks.
3. It’s easier and faster because as soon as the loan is approved by SSS, it will only take at least 24 hours for the members to withdraw it from their bank accounts.
4. The ATM-enabled UMID card is free of charge. You may directly withdraw your benefit and loan proceeds from Bancnet ATMs nationwide and VISA-branded ATMs for international transactions.
Accredited Banks for UMID ATM Cards
For SSS members to use their UMID ATM cards, they should activate them through the accredited banks.
The SSS has invited several banks to the UMID-ATM program. To date, SSS has signed agreements to implement the program with 4 banks.
UnionBank of the Philippines (UBP) is one of the accredited banks and it was the first to complete the program and started deploying kiosks in different branches:
Pioneer
Diliman
Manila
Buendia
Baguio
Cebu
Davao
Additional UBP kiosks will be available in 9 other SSS branches by yearend. And so far, about 34,000 UBP co-branded UMID cards have the ATM functionality.
While only one bank is available for now, members need not worry because the SSS is already working with other banks to join and implement the program. (Source)
UMID Cards to be received within 30 days
Many SSS members who have applied for UMID Cards in the past months have been complaining about the long waiting time for their cards to be released. That delay was due to the overheating of the machine used to produce UMID cards that caused the stoppage of operation of the Central Verification System (CVS) of the Philippine Statistics Authority (PSA).
The good news is that this issue has already been resolved and the production of UMID cards is now back to its normal processing time. It means that you don’t have to wait several months to receive your UMID Cards.
According to the Philippine Social Security System (SSS), your UMID cards will be delivered (via Philpost delivery) right to your doorsteps within 30 days upon successful application and data capture. (Source)
UMID cards are released for mailing within 8 days and will be delivered via Philpost within:
2 weeks for National Capital Region or NCR
4 weeks for those with registered addresses outside NCR

How to Verify and Follow up the Status of your UMID Application
There are 3 ways to verify and follow up the status of your UMID Card Application.
1. Through Email
To verify your application via email, you may send an email to the Identity Management Department of the Philippine Social Security System (SSS) at idcpd@sss.gov.ph.
Indicate the following information on your email message:
Complete Name
10-digit SSS number
Date of UMID Application
In the subject field of your email, indicate: Thru Facebook.
2. Through SSS Hotline
You may contact the SSS Call Center at 920-6446 to 55. Inform the SSS Representative that you’d like to verify the status of your UMID Application and provide him/her your personal information and the date of your UMID application.
3. Visit the Nearest SSS Branch
If it’s been more than a month or several months since you applied for your UMID card, you may visit the nearest SSS branch to verify or follow up the status of your UMID Application.
Beware of Scam: No such thing as Rush UMID card application with a fee
Some scammers are taking advantage of members who want to receive their UMID Cards sooner. What they do is they send a text message to the members and let them pay P100 or any amount as a fee in exchange for expediting or speeding up the process of releasing the UMID.
But the thing is, there is no such thing as “Rush UMID card application” and the SSS does not collect any fees for this matter.
This is a scam and you should not fall prey to this kind of thing. If you receive scam messages like this, report it immediately to the nearest SSS branch or call the SSS hotline at 920-6446 to 55.
You may also file a report with your own mobile phone service provider to stop the dissemination of bogus SSS text messages through their networks.
Remember, always transact directly with the SSS Personnel and/or at the SSS offices to avoid any issues.
